Our Membership

  • Unlimited extended office visits (At least 30 minutes EVERY visit).
  • Unlimited phone, text, e-mail, and videochat visits when clinically appropriate.
  • One Men’s Comprehensive Physical per year (when applicable) to review your overall health and discuss ways to improve and maintain your health over time.
  • One Well Woman Exam per year (when applicable) including comprehensive women’s health review, breast exam, pre-pregnancy counseling or counseling on birth control options, and pap smear collection at appropriate intervals.*
  • After-hours phone, text, and email visits during the evenings and on weekends are included with membership for urgent concerns. After-hours face-to-face visits are also available for urgent issues for established patients only.
  • Many additional services including contraception services (birth control), weight loss services, assistance with smoking cessation, and more.

*Lab fees for blood testing, imaging/x-rays, pap smear, and skin biopsy tissue analysis (when needed) will be deeply discounted through negotiated lab pricing. Imaging through local providers will have clear cash pricing. If you do not have insurance or choose not to use insurance to cover these, we have negotiated discount rates which are plainly listed upfront. Procedures that require additional supplies, medications, or equipment may also require a small extra fee which would be our wholesale cost for these supplies, medications, or equipment plus a small percentage to cover bottles, labels, and supplies.

†not all medicine formularies are available. We offer over 1000 of the most common medications. Controlled substances, such as narcotics or insulin are not available. We do NOT stock or dispense controlled medications at all, and do not routinely prescribe or refill controlled substance medications for long-term use. This includes medications for anxiety (like xanax and ativan), ADD/ADHD (like vyvanse, concerta, focalin, daytrana or adderall), narcotic pain meds (like percocet, vicodin, hydrocodone, etc) and insomnia (like ambien, lunesta, restoril, etc).

Can I Contact You After-hours?

Of course! Physician access is one of the greatest benefits of the DPC model.

We share after-hours calls between Dr. Jessica Fleagle and Dr. Randy Tobler. We both have similar philosophies and want to ensure that you have ready access to your physician for appropriate needs while also ensuring that we model good lifestyle practices in downtime and unplugging.

We will turn off our cell phones during protected family times: 1.5 hours each evening for family dinner time, as well as school programs and recitals, church, meditation time, etc; and will return any urgent calls or texts as soon as possible. Please be clear about your level of urgency.

We will also close the office completely for two separate weeks each year to take a break, spend time with family, unplug, relax and replenish. Don’t worry, these weeks will be announced in advance, and we will do our best to coordinate care from another physician. This will be clear on the call systems.

We may also rarely be out of the immediate area on weekends, but will still be accessible by phone, text, email, and videochat.

We use a secure EMR (electronic medical record) which is called Elation. We also utilize Spruce, HIPAA secure for texting, secure image sending, calls, and messaging. Both mechanisms allow secure telemedicine visits at your convenience. There are also secure self-scheduling options that can be seen and scheduled at your convenience.

The phone call systems and texting through Spruce are HIPAA compliant and secure, and even uploading an image on the go is secure. However, you should use your best judgment in how and when you open your information to ensure privacy in your surroundings.

Texting and email outside of Spruce are protected on our end but are considered “non-secure” methods of communication. If you wish to be able to communicate this way, you will need to authorize this during your enrollment process. Please be cautious about any photos, personal information, or private concerns that you may share via text or email as they may show up on your cell screen when we reply.

Our Billing Process

After your initial consultation, the monthly membership fees start the subsequent month. Your enrollment fee and first-month membership will be billed at the time of enrollment. If you choose to enroll mid-month, a prorated fee will be added to your billing to cover the remainder of that month until the next regular billing cycle on the first of the next month.

Billing processes on the first of each month for that month’s services and will auto-renew each month until you cancel. There are also options to pay 3, 6, or 12 months at a time. All memberships automatically renew each cycle unless you cancel in advance.

Our billing processes through the Elation EMR. Billing may show up on your bank statement or credit card, cash, or debit cards of major carriers. If you are unsure of a charge, please contact us. If charges are disputed, billing fees are added by the billing system and will be charged back to the payment account. There is a minimum 3-month membership for adults.

There is no fee for termination or cancellation. If you feel that the DPC model or Alive! Lifestyle Medicine are not right for you, cancel at any time without penalties. Any remaining full months you paid in advance will be refunded in full.
